A stretch of 0.5-3 cm of the length of the penis is retro pubic, i.e. it lies behind the pubic bone. If this tract can be pulled forward, the surgeon can obtain an extension of the outward tract. The penile extension is achieved by severing the suspending ligaments that attaches to the pubis. The penis is not actually extended; rather it is drawn forward from the body. The effectiveness varies with each patient: the extension is more obvious when the penis is small than when it is erect.From special field literature results the fact that the average extension obtained amounts to 2-2.5 cm; with 5% of the patients the surgery was of no avail due to post-surgery scar formation. In order to prevent this scar mongering tissue, the post-surgery conduct is very important. The severing of the suspending ligament yields some disadvantage: the most visible thereof is that the angle of erection shifts away; the erections come about horizontally or even in a downward position. This however does not mar the sexual performance.
